It might seem alien to some, but there are plenty of people who will work out too hard, rather than the more usual 'not hard enough'.
Week after week of training to your limit will catch up with you. You cannot train like there is no tomorrow and expect even the best nutritional habits and sleeping patterns to keep protecting you from burnout and injury. The trick is not completely avoiding the problem as such, but more of a corrective measure designed to avoid over-training and halting your progress.
To implement this I suggest you make every 5th week a lesser intensity week- reduce you weights, sets and cardio intensity by around 20%. This will allow your body to recover and keep your results coming and expect years of progressive injury free fitness and health.
